Cigar Journal Template

Ok, so I've decided to start keeping a log on cigars i've smoked. i got bored at work and made a little sheet to make copies of and put them in a 3 ringed binder. for some reason the little attachment button won't work, so if you'd like, send me a pm with your email address and i'll send it that way. also, i want someone to critique it, so let me know if you're interested.

What's the format? MS Word?

i have it in a rich text document (wordpad) and i saved it as a .jpeg.

i plan to just make a bunch of copies and hand write the info in.
